Nicotine Pouches: The New Dangerous Tobacco-Related Product Proliferating in Spain

The Basque Health Department warns about the risks of 'nicotine pouches', especially for young people, pregnant women, and those with cardiovascular issues.

The Basque Government's Health Department is alerting the public about the rise of nicotine pouches ('nicotine pouches'), a new tobacco-related product posing significant risks, particularly to young individuals.

The Basque Government's Health Department and Osakidetza have issued a warning regarding the proliferation of nicotine pouches ('nicotine pouches'), a new product linked to tobacco. These pouches are consumed by keeping them in the mouth and, in high doses, can lead to higher blood concentrations than regular cigarettes.
These pouches have well-defined cardiovascular effects and can increase the risk of fetal death in pregnant women. Contrary to the notion that these products are harmless, the Health Department and Osakidetza emphasize that there is no safe level of nicotine consumption.
Guillermo Herrero, Director of Public Health and Addictions for the Basque Government, explained that "these are pouches of varying concentrations, which people typically increase over time as a way to maintain their nicotine levels in the body, in the blood. They are usually placed between the upper lip and the gum, or slightly further back, and absorption occurs through the oral mucosa, similar to some medications, allowing nicotine to enter the bloodstream."
Herrero added, "As this practice is relatively new, there isn't much data yet regarding its health repercussions. We do know that it has the same effects as nicotine from tobacco concerning cardiovascular risks, and particularly for pregnant women, due to the potential impact on the babies through their mothers."
Consequently, three risk groups have been identified for this new form of nicotine consumption: "Children and adolescents, pregnant and breastfeeding women, as nicotine appears to cross the placental barrier and enter breast milk, and patients with cardiovascular problems."
Furthermore, among adolescents aged 14 to 18, tobacco consumption decreased from 20% in 2023 to 18% in 2025. However, this decline was offset by an increase in the use of vapes, which rose from 13% to 20% between 2023 and 2025.
"In countries where data is available, the probability of vaping among adolescents is, on average, 9 times higher than among adults. This is something we see everywhere. Moreover, the devices are designed with appealing colors and flavors, making it very easy for young people and adolescents to fall into this habit," he added.
The Basque Government reiterates that there is no safe level of exposure to tobacco, smoke, and aerosols. "All forms of consumption, both new and traditional, are dangerous for the consumer and those around them. The substances they contain and emit are toxic and can lead to serious health problems," Herrero stated.
